Community Health Plan is a not-for-profit organization offering partially self-funded and fully-insured health insurance plans and customized healthcare consulting services for employers in the Monterey Bay area. We offer the largest network of physicians and other providers on the Central Coast.
In addition to our self-funded and fully insured plans, we offer the following services:
- ERISA Health Plan Administration
We administer health plans for organizations who choose employer-sponsored, self-funded benefit plans, including plan design, TPA & reinsurance selection and interfacing, member customer service & dispute adjudication, etc.
- Care Management
Our experienced, local clinical staff provides tailored care management services to employers and their health plans. These services include utilization management, case management, preauthorization, etc.
- Disease Management
We offer a complete, customizable Disease Management program designed to improve the health of employees and health plan members. We also provide program management and administration services for organizations wishing to create their own Disease Management programs.
- Consulting
We offer consulting services in a variety of areas, including data management and reporting, predictive analytics, plan benefit review, health risk assessment, etc.
