Community HealthCare Corporation offers nationwide network coverage through its association with the Private Healthcare Systems (PHCS). Listed in this directory are only the PHCS providers representing Monterey, Santa Cruz, and San Benito counties. Please call PHCS at (800) 950-7040 or click here to visit the provider search page on the PHCS website to verify out-of-area providers (be sure to choose "Preferred Provider Organization (PPO)" as your network). If you do not see your provider on the list, please contact us with the name, phone number and/or address of the provider. We will contact that provider about joining the Guardian CHP panel.
